The All German Motorsports AGM-FCV-934H is a two-pack of replacement CV Saver discs for the hub side of a Fortin double boot flange. CV Saver discs keep joint grease from migrating out of the CV joint and into the boot.
The CV Saver kits for the Fortin double boot trans side and the EMPI double boot are interchangeable.

Working with hub side flanges
The hub side of a CV joint sits on the wheel end of the axle, where the flange connects to the hub. A flange disc like this sits between the joint and the boot and helps keep grease from escaping into the boot during operation. Because the hub side and transaxle side carry different parts, keep track of which side each disc came from during disassembly so nothing gets mixed up at reassembly.
Before reusing a disc, clean the flange face and check for grooves or heat marks. Grease that has been exposed to road debris can carry grit into the joint, so cleaning is worth doing whenever a joint is opened. Replace the disc if it is cracked or deformed, and use the same grease type the joint was packed with.
